Introduction:

Mindful Elder Care, led by geriatric care manager Robert M. Levine, LMSW, offers comprehensive services designed to provide support and resources to older adults, caregivers, and their families. Grounded in principles of mindfulness and ethical care, Mindful Elder Care aims to enhance the well-being of seniors and their loved ones through a range of tailored services.

Services Offered:

Assessment:

Cost: $160 per hour

Duration: Minimum 3-4 hours

Home Visits: Offered as an integral part of assessments to assist with care decisions.

Assessment Includes a comprehensive intake evaluation with written recommendations.

Care Management & Consulting:

Cost: $160 per hour, no minimum.

Options: Families can choose to implement recommendations independently or to continue working with a care manager in a mutually determined schedule.

The frequency of consultation and care planning is mutually based on individual needs. Sessions range from weekly to monthly, typically starting out more frequently and tapering off with check-ins as needed.

Tailored care plans outlining recommended courses of action.

Individual counseling and support for seniors and caregivers.

Facilitation of Family Meetings: mediation and coordination of family discussions regarding care decisions.

Care Coordination with Health Care Providers: liaison between clients, caregivers, and medical professionals.

Caregiver Counseling: support and guidance for caregivers navigating the challenges of elder care.

Support Group Facilitation: opportunities for caregivers to connect and share experiences.

Life Review: reflective exercises to explore personal history and experiences.

Mindful Elder Care is inspired by the teachings of Dr. Bill Thomas, Jon Kabat-Zinn, Thich Nhat Hanh, and Pema Chodron, and guided by the National Association of Social Workers (NASW) Code of Ethics. Our services prioritize both resource connection and emotional support, recognizing the unique needs of each individual and family.

Insurance Coverage:

Unfortunately, Geriatric Care Management (GCM) services are typically not covered by insurance. Advocacy efforts may be necessary in this area to explore potential coverage options or alternatives. Our commitment is to provide compassionate, holistic care that promotes the dignity and well-being of older adults and their families.
